Who this policy covers

Claimified ("Claimified", "we", "us") operates this website and the claim review forms on it. This Privacy Policy explains what we collect when you use the site or submit a claim review request, how we use it, and who we share it with.

By using this site or submitting a form, you agree to this policy. If you do not agree, please do not use the site or submit your information.

Claimified is a marketing and case acquisition company. We are not a law firm and we do not provide legal advice. See our Terms & Conditions.

Information we collect

Information you give us

When you submit a claim review form, we collect what that form asks for, which may include:

• Your name, email address, telephone number, and mailing address or ZIP code
• Your date of birth and the last four digits of your Social Security number
• Information about your potential claim — diagnosis and diagnosis date, treating facilities and physicians, exposure history, employment history, and similar details
• Documents you choose to upload, such as a photo ID or proof of medication
• Any other information you type into a comment or message field

Some of this is sensitive information, including health-related information. You choose whether to provide it. Provide only what you are comfortable sharing.

Information collected automatically

• Your IP address, browser type, device type, and operating system
• Pages you view, links you click, and the date and time of your visit
• The page or advertisement that referred you to us

Consent records

When you submit a form, our consent-verification provider (ActiveProspect TrustedForm) records the page as you completed it and issues a certificate. That certificate documents the consent you gave and is retained as proof of it.

How we use your information

• To review your submission and evaluate whether your potential claim may qualify
• To contact you about your submission by phone, text message, and email
• To connect you with a participating law firm or attorney who may be able to evaluate or handle your claim
• To verify your identity and the accuracy of what you submitted
• To maintain records of the consent you provided, as required by law
• To operate, secure, measure, and improve the site and our advertising
• To comply with legal obligations and to establish or defend legal claims

How we share your information

We share the information you submit with:

• Participating law firms and attorneys, and their intake staff, so they can evaluate and potentially represent you
• Authorized marketing partners and lead purchasers who work with those firms on the case type you inquired about
• Service providers who operate parts of our process on our behalf — including lead delivery and verification platforms, email and messaging providers, hosting providers, and analytics and advertising platforms
• Authorities and other parties where required by law, subpoena, or court order, or to protect our rights, safety, or property
• A successor, if Claimified is involved in a merger, acquisition, or sale of assets

We do not sell your information to parties unrelated to the claim review you requested.

Calls, texts, and emails

When you check the consent box on a form, you give your express written consent for Claimified, its participating law firms, and its authorized marketing partners to contact you at the phone number and email address you provided — including by autodialed and prerecorded or artificial-voice calls and by automated text message — even if your number appears on a federal or state Do Not Call registry.

Consent is not a condition of receiving any service. Message and data rates may apply.

Withdrawing consent

• Reply STOP to any text message to stop texts
• Use the unsubscribe link in any email to stop emails
• Ask the caller to place you on the internal Do Not Call list
• Email Claimified01@gmail.com or call (701) 997-1892 to withdraw consent entirely

Withdrawing consent stops future marketing contact from us. It does not by itself withdraw your information from a law firm that has already received it — contact that firm directly.

Cookies, pixels, and advertising

This site uses cookies and similar technologies, including the Meta (Facebook) Pixel, to measure how our advertising performs and to show ads on other platforms to people who visited the site. These tools may set cookies and share information such as your IP address and the pages you viewed with the platform that provides them.

You can block or delete cookies in your browser settings, and you can adjust ad personalization in your Meta account settings. Blocking cookies may affect how parts of the site work.

We do not currently respond to browser "Do Not Track" signals.

How long we keep information

We keep claim review submissions and the consent records that go with them for as long as needed for the purposes described in this policy, and afterwards for as long as required to comply with legal, tax, and recordkeeping obligations or to establish or defend legal claims. Consent records are typically retained for at least the period set by applicable telemarketing rules.

How we protect information

We use reasonable administrative, technical, and physical safeguards to protect the information we hold, including encrypted connections for form submissions. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

Please do not email us documents containing sensitive personal information; use the upload fields on our forms instead.

Your privacy rights

Depending on where you live, you may have the right to:

• Know what personal information we have collected about you and how we use and share it
• Request a copy of that information
• Request that we correct inaccurate information
• Request that we delete your information
• Opt out of the sale or sharing of your personal information for cross-context behavioral advertising
• Limit the use of sensitive personal information
• Not be discriminated against for exercising these rights

Health-related information

Claimified is not a health care provider, health plan, or health care clearinghouse, and it is not a HIPAA-covered entity. Information about your diagnosis or treatment that you submit through this site is handled under this Privacy Policy rather than under HIPAA. A law firm or provider you separately authorize to obtain your medical records will handle those records under its own obligations.

Children's privacy

This site is intended for adults. We do not knowingly collect personal information from anyone under 18, and our forms should only be completed by someone 18 or older. A parent or legal guardian may submit information about a minor's potential claim on the minor's behalf. If you believe a child has provided us information directly, contact us and we will delete it.

Other websites

Our site links to websites we do not operate. We are not responsible for the privacy practices or content of those sites. Read the privacy policy of any site you visit.

Changes to this policy

We may update this Privacy Policy. When we do, we will change the "Last updated" date at the top of this page. Material changes will be posted here, and continuing to use the site after a change means you accept the updated policy.
